You confirm that all information provided by you when entering into your Membership Agreement or using our facilities is correct, accurate, complete and up to date and may be relied upon by Abbey Sports.

Responsibility for Your Health

You are responsible for ensuring that you are medically fit and capable of participating in exercise and physical activity. To the best of your knowledge and belief, you are in good health and are not knowingly incapable of engaging in active or passive exercise in a way that would be detrimental to your health, safety, comfort, wellbeing or physical condition.

Medical Advice

You acknowledge that Abbey Sports staff, agents and subcontractors are not medical professionals and cannot provide medical advice. If you have any concerns regarding your health, fitness or ability to exercise safely, you should seek independent medical advice before undertaking physical activity at Abbey Sports.

Medical Conditions and Changes to Your Health

You must inform a member of staff on your first visit if you have a medical condition, disability, injury or physical limitation that may affect your ability to exercise or use our facilities safely. You must also notify us of any new or changed medical condition, injury or physical limitation that may affect your ability to exercise safely before continuing to exercise.

Health Declaration

To the best of your knowledge and belief, you confirm that:

By checking the box, you confirm that you have read, understood and agree to the following:

I understand that if I am unable to agree with any of the statements above, have any concerns regarding my health or fitness, or my circumstances change, I should seek appropriate medical advice where necessary and notify Abbey Sports before continuing to exercise. I acknowledge that Abbey Sports staff are not medically qualified and cannot provide medical advice.

Exercising Within Your Capabilities

You must never exercise beyond your own capabilities. You agree to stop exercising immediately and inform a member of staff if you feel unwell or experience dizziness, unusual pain or discomfort, chest pain, unusual shortness of breath or any other adverse symptoms while exercising.

Changes in Circumstances

If any part of this Health Commitment Declaration ceases to be true or your health or vulnerability to injury changes during your membership, you agree to notify Abbey Sports and, where appropriate, seek independent medical advice before continuing to exercise.

Using Equipment Safely

You will seek assistance from a member of staff if you are unsure how to safely operate or use any equipment or facilities provided by Abbey Sports.

Respect for Our Facilities

You agree to use Abbey Sports equipment and facilities responsibly, respect the equipment provided and behave in an appropriate manner at all times when using our facilities.

Exercise Risks

You acknowledge that exercise and physical activity carry inherent risks. These may include injury, illness or other adverse health effects. You agree to exercise responsibly, within your own abilities and with regard to any medical advice you have received.

Disclosure of Relevant Information

You understand that failure to disclose relevant information about a medical condition, injury, disability or physical limitation may affect our ability to provide appropriate assistance or make reasonable adjustments for your safe use of the facilities.

Unable to Make This Declaration

If you are unable to confirm any of the declarations above, you must notify a member of Abbey Sports staff before undertaking exercise or using equipment or facilities so that appropriate next steps can be considered.

Gym Induction

Abbey Sports offers members the opportunity to receive an induction and guidance on the safe use of gym equipment.

Unless you actively choose to book a complimentary gym induction, you acknowledge that you are opting out of this induction.

Choosing not to undertake an induction does not remove your responsibility to seek assistance from a member of staff whenever you are unsure how to safely use any equipment or facility.

Disability, Accessibility and Additional Support

There are many different disabilities, medical conditions, injuries and impairments that may affect how an individual uses our facilities.

If you consider yourself to have a disability, require reasonable adjustments or need additional assistance to use our facilities safely, please notify Abbey Sports as soon as possible.

Where appropriate, we will discuss your requirements with you and take reasonable steps to support your safe use of the facilities. This may include an appropriate induction, consideration of reasonable adjustments and information regarding relevant emergency and evacuation procedures.
